Henna Art Permission Form

Henna designs are temporary and last between 1 – 3 weeks.

Henna is made from a flowering plant grown in tropical and subtropical regions. Henna paste is applied to the skin in various patterns, leaving a temporary red-brown stain. This art form has been practiced for centuries in India, Africa, and the Middle East. During this program, henna will be applied to participants’ skin (hands, arms, or ankles, feet) to create a temporary henna art design. The design is not permanent, but may last as long as three weeks. Participants will receive one design. If your child has any allergies, please take special consideration before signing this form.

RELEASE AND INDEMNITY AGREEMENT

For and in consideration of the permission of the Huntley Area Public Library District (hereinafter referred to as the “Library”) allowing my minor child to participate in the Henna Art program, I hereby release and discharge the Huntley Area Public Library District, and its officers, employees and agents (hereinafter referred to collectively as the “Released Parties”), from all claims of any kind of character which I, or my minor child have or might have against the Library or the Released Parties, and especially because of all damages, losses or injuries to person or property, or both, resulting or to result from any accident involving my myself or my family members during the said Henna Art program.

As additional consideration for the foregoing permission, I expressly stipulate and agree to indemnify and hold harmless forever the Library and the Released Parties against any and all claims, demands or actions which may at any time be made or instituted against the Library or against the Released Parties by my said minor children or by me, or by any guardian or legal representative of my said minor child, or by anyone else on his or her behalf, for the purpose of enforcing any claim for damages on account of injuries to person or property, or both, sustained by my family members during the course of the said Henna Art program.
